Mounjaro on the NHS, and what the UK price means

In England, NHS services can use Mounjaro for weight management, and access comes in stages. Specialist services start at BMI 35 plus one weight-related condition. For primary care in 2026/27, Cohorts I and II need BMI 35 plus four listed conditions. NICE lists £133–£330 for four weeks and excludes VAT. That is not what the NHS pays or one set price for private patients.

Meeting the rules does not guarantee a prescription. A clinician decides whether the drug is right for the patient. The local service must also have a way to assess and support the patient. These obesity rules are separate from those for type 2 diabetes.

NICE publishes four price points for six KwikPen strengths. These are list prices. They are not private offers or the price paid by the NHS.

List price

NICE states these are list prices for a four-week supply of pre-filled pens, excluding VAT.

NHS price

The NHS purchase price is commercial in confidence, so the amount the NHS pays is not published in the guidance.

Private price

A private prescription requires a clinical consultation, and the final private patient price varies by prescriber and dispensing pharmacy. No national private retail price exists.

£9.90 per item unless exempt. That is the NHS prescription charge per item, not Mounjaro’s drug price. NHS exemptions still apply.

Two access settings, two thresholds

Specialist services and primary care do not use the same gate. NHS England expands primary care by cohort. During 2026/27, that includes the original Cohort I and the new Cohort II.

The five qualifying conditions

NHS England says the BMI threshold is usually reduced by 2.5 kg/m² for people from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds.

This rollout table is for NHS England. Scotland, Wales and Northern Ireland run separate health services with their own rules.

Who will get Mounjaro on the NHS?

In an NHS England specialist service, the NICE cohort starts at BMI 35 plus one weight-related condition. The 2026/27 primary-care rollout is narrower. It starts at BMI 35 plus four listed conditions, with the adjustment shown above.

Can I ask my GP for Mounjaro?

You can ask how your local NHS pathway works. NHS England funds primary-care access in phases. Local integrated care boards set up the assessment route. Meeting the cohort does not require a clinician to prescribe it.

What does Mounjaro cost in the UK?

NICE lists £133 to £330 for four weeks, based on strength and excluding VAT. The NHS price is confidential. In England, a patient who is not exempt pays £9.90 per item. Private prices vary, so one seller’s offer is not treated as a national price.

What remains uncertain

  • It does not apply NHS England’s rollout rules to Scotland, Wales or Northern Ireland.
  • It does not treat NICE’s list price, the confidential NHS price and a private patient’s bill as the same number.
  • It does not say that meeting a BMI and comorbidity threshold guarantees assessment or a prescription.
  • It does not use the obesity pathway to answer the separate type 2 diabetes criteria.
