Cagrilintide is a weekly shot in weight-loss studies. It copies amylin, a natural signal that helps you feel full after eating. Sellers offer unapproved cagrilintide for weight loss, sometimes paired with semaglutide under the name CagriSema.
A phase 2 trial found more average weight loss at higher doses. It has moved into phase 3 trials with semaglutide. Cagrilintide does not have FDA approval.

Large draws, because the milligrams are large
Cagrilintide fills more of the barrel than most things here. That says how much powder the dose is. It says nothing about how strong the dose is.

It copies a different hormone from its neighbors
The compounds either side of this one on the site copy gut hormones. Cagrilintide copies amylin, which the pancreas releases to end a meal. It sits here because you mix it the same way, not because it behaves the same way.
The practical consequence is on the scale. Its studied range reaches higher in milligrams than semaglutide’s does. A dose that sounds alarming beside a semaglutide figure is ordinary here, and comparing milligrams across compounds compares nothing.
A long draw is a dilute vial, not a big dose
A plunger drawn most of the way up the barrel looks dramatic beside a research peptide’s four or five units. It only means the powder is spread through a lot of water.
Halve the water and the same dose reads half as far up. Identical milligrams, identical injection, different picture. The number of units is a fact about your mixing and nothing else.
The mistake: reaching for the larger water volume by habit
More water is the usual advice on this site. It spreads small doses over more marks. Cagrilintide’s doses are not small.
With the smaller vial and the larger volume, the top of the studied range runs past the end of a U-100 barrel. It then has to come out in two pulls, and two pulls is two chances to misread one dose. Here, less water keeps a dose in a single draw.
The starting quantity is the arm that moved on, and it did not move on alone
The number the tool loads with is the arm the developers carried into their later trials. Those trials do not study it by itself. They study it combined with another compound, in one injection, under a name of its own.

How the math works
Concentration — divide the amount in the vial by the water you added: mg ÷ mL = mg/mL. This tells you how many mg are in each mL.
Volume — the dose divided by that concentration: dose ÷ (mg/mL) = mL.
Units — on a U-100 syringe one unit is 0.01 mL, so mL × 100 = units. A U-40 unit is 0.025 mL, so the wrong barrel is a 2.5× error. That is why the calculator asks which one you have.
Rounding gap — the exact draw against the nearest syringe line: |exact − nearest line| ÷ exact × 100 = % difference.

30 mg is more powder than either cagrilintide vial here holds, and the operation is the same one. Divide milligrams by milliliters: 30 mg in 1 mL is 30 mg/mL, in 2 mL it is 15, in 3 mL it is 10.
At 30 mg/mL one U-100 unit carries 300 micrograms. The 2.4 mg carried into phase 3 would be 8 units, and a single misread mark moves it by 300 micrograms. Strong solutions punish small errors.
How much water to add to reconstitute peptides?
More water never changes the milligrams. It spreads them through more liquid, so the same dose becomes a longer draw that is easier to read.
Cagrilintide is where that advice meets its limit. Its studied range reaches 4.5 mg, and the 5 mg vial in 2 mL puts that at 180 units — nearly two full barrels. Here the smaller volume is what keeps one dose in one pull.
How many units of cagrilintide?
It depends on the vial and the water, and this page’s four pairings give four answers for one dose. The 2.4 mg carried into phase 3 draws 24 units from 10 mg in 1 mL. It draws 48 from 10 mg in 2 mL or 5 mg in 1 mL, and 96 from 5 mg in 2 mL.
All four are the same injection; only the liquid around it changed. A unit count quoted without its vial size and its water volume describes somebody else’s glass.
How to mix cagrilintide with tirzepatide?
No published study has given cagrilintide and tirzepatide to anyone together. The partner cagrilintide actually went into phase 3 with is semaglutide, co-formulated in one injection as CagriSema.
When two peptides share a vial, whoever filled it fixed the ratio. Moving the plunger moves both at once, so you set one amount and simply receive the other. That constraint is arithmetic, and it says nothing about whether the pair has been tested.
