The short answer

A milligram of semaglutide has no fixed unit count. Divide the vial strength by the water to get the strength of the liquid. Divide the dose by that figure, then by 0.01 for a U-100 barrel. At 5 mg in 2 mL, one unit holds 25 mcg, the starting 0.25 mg is 10 units, and the full 2.4 mg is 96 units.

Every one of the five steps lands on a syringe line at that mix. Change the water and that stops being true.

The problem here is a short draw, not a missed mark

Semaglutide doses are small. The first step is 0.25 mg — under a tenth of where the schedule ends. A small dose covers few marks, and few marks is where reading goes wrong.

At the opening mix the first dose is 10 units, so one mark is 10% of it. The last dose is 96 units, where the same mark is 1.0%. One slip of the plunger costs far more at the start of a schedule than at the end of it.

Halve the water and it gets worse. In 1 mL the liquid is 5.00 mg/mL, and the first dose shrinks to 5.0 units. More water is the lever, and it costs nothing.

Ozempic and Wegovy clicks are not these units

Ozempic and Wegovy are semaglutide, and they follow this schedule. Neither one is a vial you draw from. An Ozempic pen has a dose dial that clicks, and a click is a step on that dial. It is not a mark on a barrel.

So a click chart and this chart answer different questions about different objects. We work out marks on a syringe, and we do not model pen clicks. If you are counting clicks, nothing on this page converts to them, and no arithmetic here should be used as if it does.

Questions people ask about semaglutide units

How many units is 0.25 mg of semaglutide?

Only the vial can answer that. At 5 mg in 2 mL it is 10 units. In half that water it is 5.0 units — the same dose, drawn on a different part of the barrel. Put your own two numbers in above.

What does 20 units of semaglutide carry?

That depends on the vial it came out of, and a unit count on its own does not name one. Set the vial and the water above, then read the lower panel. It also shows what the same count would carry on a U-50 and a U-40 barrel, because the count alone never says which was meant.

Should I add more water or less?

More water spreads the same drug over more marks, which makes a small dose easier to read. Less water makes the draw shorter. The chart shows what each choice does, and the vial holds the same amount of drug either way.

Is compounded semaglutide the same strength as a pen?

Usually not, and that is the whole reason to check. A compounded vial is labeled with its own strength in milligrams and its own volume. Read both off the label rather than assuming a pen figure carries over.

Why is the last dose almost the whole barrel?

Because a thin liquid needs more volume to carry the same drug. At the opening mix the 2.4 mg step is 96 of the 100 marks on a U-100. Less water brings it down the barrel, at the cost of making the first step shorter.
