Do GLP-1 pens come with needles?
2 of the 4 presentations here arrive ready to inject. The other 2 expect you to supply the needle. The device decides it, not the brand.

What matters first
Two products in this set ship in more than one form, and the answer changes between those forms. So the brand on the box is not what settles it. The device inside is.
What each label says
Read down the middle column. A vial almost never includes anything, and a pen usually does — except for the one that is refilled with a fresh needle every week.
| Product | Device | Needle in the box | What the label says |
|---|---|---|---|
| Zepbound | Vial | No | The carton prints “Needles and Syringes are not included”. The label asks for a 1 mL syringe that can measure a 0.5 mL or 0.6 mL draw, and for a new syringe and needle each time. |
| Zepbound | Single-dose pen and KwikPen | Yes | The pen carries its own needle. The label’s steps never mention attaching one, and it says not to move the medicine from a pen into a syringe. |
| Wegovy | FlexTouch single-patient-use pen | No | The label says to always use a new needle for each injection, to remove and discard the needle after each one, and to store the pen with no needle attached. |
| Wegovy | Single-dose pen and single-dose syringe | Yes | The label lists cartons of four single-dose pens and cartons of four single-dose syringes with integrated needles. Nothing separate is needed for either. |
What needle size do the labels specify?
None of them. Not one label in this set states a gauge or a length for the needle you supply. The most specific of them gets as far as naming the barrel.
That is an uncomfortable answer to a question asked several thousand times a month, and it is the true one. Search results are full of confident numbers for this, and none of them can be traced to a label, because the labels do not carry them.
Every label in the set does state two things. A fresh needle goes on for each injection. And needles and pens are never shared between people. Those two are unanimous across all four documents.
What the labels do settle: which barrel, and what a mark on it is worth
Why the vial version needs more from you
A pen holds a mechanism that measures the dose. A vial holds liquid. Everything the pen does for you has to be done with a syringe instead, and the syringe is not supplied.
This is not a small difference and our own report-stream study measured it. Across the FDA side-effect data, the drugs people draw from a vial carry far more dosing-error reports than the ones that arrive in a dialed pen. The arithmetic a vial demands is where those reports come from.
