What does Foundayo cost if your plan covers it?

The $25 offer is for eligible adults whose commercial plan covers Foundayo. Lilly caps its payment at $100 for one month, $200 for two months, or $300 for three months. The yearly cap is $1,000 across no more than 10 fills, and the card expires on 2026-12-31. If the plan does not cover Foundayo, prices and savings caps vary by dose. Both offers require a prescription for a use covered by the FDA label.

What does Foundayo cost without insurance?

Lilly’s self-pay terms publish six dose-specific prices for a 30-day supply. The starting 0.8 mg dose is $149; the 9 mg dose is $299. Two middle doses are temporarily reduced through the end of 2026.

At 14.5 mg and 17.2 mg, $299 is a purchase offer with a 45-day refill clock. If the next purchase happens later, Lilly says the regular price is $349.

The $649 figure is separate from the self-pay table. Lilly calls it the wholesale acquisition cost for 30 tablets. It is a distribution price before rebates and other changes, not an offer to a patient.

The catch on that price

This is Lilly’s separate Self-Pay Savings Card program. It permits up to 10 fills per calendar year and expires on 2026-12-31. The monthly savings cap is $520 at 0.8 and 2.5 mg, $470 at 5.5 mg, and $370 at 9, 14.5, or 17.2 mg. The first $299 purchase at 14.5 mg or 17.2 mg is automatic. Keeping that price requires the next purchase within 45 days of the previous delivery or receipt.

The program is for cash-paying patients and runs outside insurance. Lilly says you cannot seek or accept reimbursement from private insurance, a government program, or a healthcare reimbursement account. You also cannot apply those costs toward a deductible or true out-of-pocket requirement.

What if you are on Medicare?

Lilly lists Foundayo at $50 a month through the Medicare GLP-1 Bridge for eligible Part D patients with a prescription for weight management.

Eli Lilly and Company sets conditions beyond the exclusion above, and you attest to all of them when you enroll. Getting one wrong is what turns a $25 month into a full-price one.

  • You are 18 or older and live in the United States or Puerto Rico.
  • You have commercial drug insurance and a prescription for a use the FDA label covers.
  • You are not enrolled in a state, federal or government-funded healthcare program.
