L-carnitine shots: what the name does—and does not—tell you

“L-carnitine shot” does not name one standard treatment. The approved injection reviewed here goes into a vein for certain carnitine deficiencies. Wellness clinics may sell different compounded shots or mixtures. Their routes, ingredients and evidence are not the same.

What the current label covers

The approved product is an IV deficiency treatment

The label covers certain carnitine deficiencies linked to inherited metabolic problems. It also covers deficiency in people with end-stage kidney disease who receive dialysis.

Why route matters

A wellness shot does not inherit the IV label

The approved injection reviewed here goes into a vein. A compounded shot given into a muscle or under the skin is a different product-and-route question.

When ingredients are combined

L-carnitine added to semaglutide

FDA says some compounders add levocarnitine and other ingredients to semaglutide. FDA also says the safety and effectiveness of those combinations have not been established. The evidence for one ingredient cannot prove the whole mixture works better.

One name is not one formula

FDA has named L-carnitine and mixtures separately

A 2019 FDA warning listed an L-carnitine injection, MIC products and other mixtures as separate unapproved products. The warning applied to those named products. It does not show that every clinic shot contains the same ingredients or has the same quality problem.
