What does Zepbound cost if your plan covers it?
Zepbound’s commercial offers depend on the named presentation and its coverage. An eligible person whose commercial plan covers the single-dose pen can pay as little as $25, with savings capped at $100 for one month. For eligible commercial plans that do not cover that pen, the offer is as low as $499 for one month. A separate non-covered KwikPen offer varies by dose and has its own purchase conditions. The table keeps those offers separate.
The $100 limit is the most the card contributes to a covered one-month pen fill. It is not a limit on what the patient pays. That distinction matters when comparing the advertised $25 price with a pharmacy quote.

What does Zepbound cost without insurance?
Lilly sells the KwikPen and the vials from $299 a month through its self-pay offer. The single-dose pen is not included in that offer.

Requirements for this price
The $449 offer applies automatically to your first purchase of 7.5 mg or higher. To keep it, complete the next purchase within 45 days of the previous fill’s delivery or receipt. A later purchase returns to the regular price: $499 at 7.5 mg or $699 at 10 mg and above.
The $449 purchase offer on 10 mg and above depends on refill timing. Each refill must be bought within 45 days of the last delivery. After that deadline, the regular price is $699.
This self-pay program runs outside insurance. The $299 starting price covers the 2.5 mg KwikPen or vials. It does not apply to the single-dose pen.

What if you are on Medicare?
Medicare covers Zepbound under the GLP-1 Bridge at $50 a month, but only the KwikPen. Medicare’s page says the program does not cover single-dose Zepbound vials or pens.
The Medicare GLP-1 Bridge lists Zepbound KwikPen at $50 a month for eligible participants. It excludes Zepbound single-dose vials and pens. The prescription’s presentation and the program’s other eligibility rules both matter.
The Medicare GLP-1 Bridge runs from 2026-07-01 to 2027-12-31, at $50 a month. Two things about it cost money and are easy to miss.
- The Part D deductible does not apply, and no part of the $50 counts toward your out-of-pocket limit.
- There is no low-income subsidy on it, so people who get help with drug costs may pay more here than under their plan.
