What Zepbound costs, and who each price is for
A month of Zepbound costs $25 if you have commercial insurance that covers it, and $299 to $699 if you pay cash. Medicare and Medicaid are shut out of the $25 card.

Every price here was read from the manufacturer’s own program terms on 2026-08-12. Those terms say they can change at any time, without notice. The terms themselves state that they expire on 2026-12-31 — after that date, treat every figure on this page as history until it has been read again.
What does Zepbound cost if your plan covers it?
Lilly runs two different Zepbound cards, and which one you get depends on a detail most people never think to check: whether your plan covers the pen or the KwikPen. If your plan covers the single-dose pen, the card takes you to $25 and pays up to $100 toward it. If your plan covers nothing, the same card floors you at $499 instead. And if your plan covers the pen but not the KwikPen, you land on a third ladder priced by dose. Those are three different answers to one question, and the card sorts you into one of them without asking.

What does Zepbound cost without insurance?
The cash lane is where Zepbound gets cheap. Lilly sells the KwikPen and the vials from $299 a month. That undercuts what plenty of insured people pay before a deductible is met. The single-dose pen is not in that offer at all.
The trap is the refill clock. The $449 price on 10 mg and above is a purchase offer rather than a price. It holds only while each refill is bought inside 45 days of the last delivery. Let it lapse and the same box lists at $699. Two weeks of delay costs $250.

The catch on that price
The $449 arrives automatically on your first purchase of 7.5 mg or higher. To keep it, Lilly asks you to buy the next one within 45 days of the last one being delivered. Buy it later and the offer does not apply, which at 10 mg and above is $250 more.
Self-pay runs outside insurance. Lilly prices it the same way whoever you are, and the $299 headline is for the KwikPen or the vials, not the single-dose pen.
The cheap Zepbound is not the same object as the expensive one. The $299 price covers the KwikPen or four vials. The single-dose pen is left out of it. So the saving arrives attached to a device you draw a dose from, rather than one that arrives set. That is a trade worth seeing before you take it.
What if you are on Medicare?
Medicare covers Zepbound under the GLP-1 Bridge at $50 a month, but only the KwikPen. Medicare’s page says the program does not cover single-dose Zepbound vials or pens.
Zepbound is the one Lilly product Medicare will help with, and only in one form. The Bridge covers the KwikPen at $50 a month. It leaves out the single-dose vials and pens. So a Medicare reader holding a prescription for the pen has a device problem rather than a price problem. That is a question for the prescriber.
The Medicare GLP-1 Bridge runs from 2026-07-01 to 2027-12-31, at $50 a month. Two things about it cost money and are easy to miss.
- The Part D deductible does not apply, and no part of the $50 counts toward your out-of-pocket limit.
- There is no low-income subsidy on it, so people who get help with drug costs may pay more here than under their plan.

What does a pharmacy pay for Zepbound?
Zepbound is the one product here where the device changes the number, and it changes it by a third. Four single-dose pens and one KwikPen deliver the same four weekly doses of the same drug. The KwikPen costs a pharmacy far less to buy.
That lines up with everything else about it. The KwikPen is the device Lilly’s cash price covers, and the only Zepbound form Medicare will help with. The cheap route is cheap the whole way down rather than being a promotion held open for a while.
The KwikPen is also the only Zepbound ladder in this file where the dose moves the price at all: the three lower doses sit in one band and the three higher ones in another.

Read down the single-dose pen table and the dose stops mattering. Every strength sits within 0.05 percent of every other, from $1,052.00 to $1,052.55 a month. What a pharmacy pays does not climb as the dose does.
What this number is not
- It is an acquisition cost, not a patient price. What you pay is set by your plan, your pharmacy and any manufacturer program you qualify for, and none of those are in this file.
- CMS prices these products per milliliter or per tablet. Any monthly figure on this site is our arithmetic on top of that, using the package size from the product’s own label, and it is labeled as derived wherever it appears.
- Wholesalers, pharmacy benefit managers, plans and manufacturers settle further amounts between themselves that are not public. A difference between what a pharmacy pays and what anyone is charged is not evidence of wrongdoing, and this site does not present it as one.

Eli Lilly and Company sets conditions beyond the exclusion above, and you attest to all of them when you enroll. Getting one wrong is what turns a $25 month into a full-price one.

- For the KwikPen offers, your commercial plan must NOT cover the KwikPen.

What makes Zepbound different on price
Zepbound is the only one of the four where the cash price depends on how fast you refill. Lilly holds the higher doses at $449 a month. That price survives only if the next refill is bought within 45 days. Miss the window and the same box is $699.
Zepbound is tirzepatide from Eli Lilly and Company, approved under NDA 217806. Its price ladder tracks its dose ladder, which starts at 2.5 mg and stops at 15 mg. That is why a quote taken in your first month is a quote for a dose you are meant to leave.
