This category is unusual on this site because the evidence is not thin. Thymosin alpha-1 is an approved medicine in several countries and carries a trial record running to thousands of patients. The problem is what those trials were about.
They studied acutely unwell hospital patients — hepatitis B, liver failure, sepsis, melanoma — not healthy adults wanting resilience. Each row gives the practice, then what the trials covered.

Three compounds, and who was enrolled
Read the right column for how ill the participants were. Every trial here enrolled hospital patients, which is not who buys these for immune support.

A large trial that answers the question is worth more than a small one that flatters it
The sepsis trial is the most valuable single result in this category. It enrolled 1,106 people and ran double-blind against placebo. By day 28 it reported 23.4% mortality on the drug against 24.1% on placebo. That is a real answer to a hard question, and the answer is no.
A seller will not quote it, because a negative phase 3 is the opposite of a testimonial. A reader weighing whether to spend money on this compound should want it most of all. It is the only study here big enough to settle anything.
The approval is also real and it is worth reading precisely. An approval granted elsewhere carries that jurisdiction’s review standard and its permitted claims, not America’s. There is a practical gap too. The trials dissolve 1.6 mg of powder in 1 mL and inject that. Research vials under this name hold 5 mg or 10 mg.
Nobody has studied a healthy adult
Every trial on these three records enrolled people who were already ill — hepatitis B, acute-on-chronic liver failure, sepsis, metastatic melanoma, venous leg ulcers. Immune support in a well person is not an endpoint any of them measured.
That is a different statement from the compound not working. It is that nobody has asked the question people buy it to answer. The trials that exist tell you what happens when a very sick person receives it, which is useful and is not the same thing.
